The homicide of Tina Fontaine's father in 2011 "profoundly impacted" her life and every system she came into contact with afterwards failed her, according to a report released Tuesday by the Manitoba Children's Advocate.

Police said they were investigating a "serious incident" on Nairn Avenue by Allan St. on Wednesday. They later confirmed an adult male had died.
