According to WorkSafeBC, Shane Gorner, 19, was killed on the job when he was leaning between the plates of a rubber-block press and the plates unexpectedly closed.
Prior to the incident, WorkSafeBC had cited the company’s power presses were unsafe for operation and didn’t have the proper safeguards to protect workers from injury.

Furthermore, WorkSafeBC concluded instead of the company fixing the unsafe machines, “the firm had instructed workers to use a work procedure that required them to enter the pinch point of the press.”
