Searching for accountability in Chinatown stabbing

Why was a psychiatric hospital patient who stabbed his daughter to death allowed to be unsupervised in Vancouver, and what led to that decision? It’s been three days since the triple stabbing in Chinatown, and while the accused is in custody, those responsible for allowing him to be unescorted are still not providing answers.
