BC Hydro CEO Chris O’Riley apologized to the public on Thurs. June 22, 2023, after an independent report found that the Crown corporation’s procedural gaps contributed to an underground electrical vault explosion in downtown Vancouver in February that left two people with serious injuries. The investigation also found that safety risks had previously been identified with the Burrard Street vault, but a recommendation to replace it was never followed.
