Mass Casualty Commission commissioner Leanne Fitch spoke Thursday about some of the findings of the inquiry into the Nova Scotia mass shooting, saying “there were failures” by RCMP. She said in addition to failures in communication during and in the aftermath of the tragedy, she said there were also “many warning signs of the perpetrator’s violence” and “missed opportunities” to intervene before the shooting.
