Metro Vancouver officials say ‘human error’ caused deadly malfunction at Cleveland Dam

A preliminary Metro Vancouver investigation blames human error for the deadly malfunction last Thursday at the Cleveland Dam. A father and son from North Vancouver died in the flash flood while fishing on the Capilano River.
