On Oct. 29, 2014, the worker was on a zoom boom that was extended around 25 to 30 feet in the air when it was tipped by a falling tree. The man suffered several broken bones.

OHS officials said employers and contractors are required to notify them if a worker sustains an injury that requires them to be in hospital for three days or more.
