Insurance claim denied: company won’t pay widow $12,000 citing inaccurate form

A Delaware, Ontario widow can’t collect a $12,000 life insurance policy taken out by her husband because the company claims the form wasn’t filled out properly. As Seán O’Shea reports, one lawyer says denials like these are common.
