Indonesian authorities have released their final report into the crash of a Boeing 737 MAX jet in the Java Sea one year ago. It shows there were a series of failures by everyone from Boeing designers and engineers, to Lion Air, its pilots, and even spare-parts suppliers. Redmond Shannon reports.
