A Philadelphia bus driver is facing disciplinary action after falling out of her seat Tuesday and narrowly missing a freight train during a crash.

Dramatic surveillance footage shows the unidentified 10-year SEPTA (Southeastern Pennsylvania Transportation Authority) veteran attempt to make a U-turn before sliding out of her seat and falling to the floor. Passengers inside the bus were tossed around as the bus jumped a divider and ended up on train tracks – narrowly avoiding a freight train.
The driver was allegedly not wearing her seat belt and was eventually brought to hospital, along with two female passengers, with non-life-threatening injuries, according to a local NBC affiliate.

“We believe that this accident would not have occurred if she had her seat belt on,” said SEPTA spokeswoman Jerri Williams.
