An independent investigator has made more than 100 recommendations in a report on deaths at the Lakeshore General Hospital’s emergency room. The investigator met with close to 70 people and describes the ER as being entrenched with bad habits where communication problems are critical. The work environment is considered toxic with little accountability on behalf of staff. As Global’s Phil Carpenter reports, there were six reported deaths in the hospital’s ER since 2019.
