WINNIPEG – A southwestern Manitoba business is facing a dozen charges under the Workplace Safety and Health Act for a fatal incident two years ago.
Dale Jerome was working for Cobalt Industries in Carberry in May 2012. The 22-year-old died when a steel catwalk panel he was lifting with a hoist fell on him.

Cobalt Industries was known as Fusion Design and Fabrication Ltd. at the time of the incident.
Among the allegations the company faces is that it failed to ensure Jerome was protected from the falling catwalk panel.
It’s also accused of failing to provide workers with a hand chain hoist that was safe and did not provide workers with adequate training and supervision while the hoist was used to build the catwalk.
