BC Coroners looking into three mental health deaths following release from Abbotsford Hospital

A public inquest will be looking into the deaths of three patients who died shortly after leaving Abbotsford Regional Hospital.

The BC Coroners Service made the announcement today and said the three patients had been admitted to the hospital for mental health issues within days of their deaths.

Maple Ridge resident Brian David Geisheimer, 30, died in Mission on Dec. 28, 2014; 19-year-old Sebastien Pavit Abdi died at his Abbotsford home on Apr. 26, 2015 and Sarah Louise Charles, 41, died the same day near her Abbotsford apartment building.

Coroner Liana Wright and a jury will hear evidence surrounding the three deaths and make some recommendations to prevent further similar deaths.

The B.C. Health Ministry said in a written statement to Global News that when a patient passes away unexpectedly, it’s a matter of “serious concern” to the health authority and ministry. The statement goes on to say:

We always strive to learn from incidents like these, and Fraser Health is reviewing the details of these cases, and will be cooperating fully in the Coroner’s inquest. All health authorities and the Ministry of Health work closely with the Coroners Service, and respond to all requests for information and documentation.

Fraser Health will respond to any recommendations made by the Inquest, and has internal review processes which are being followed in relation to each of these events.

The inquest is set for May 16, 2016 at the Burnaby Coroners Court.
