Lisa Hughes works with the Alzheimer’s Society in Peterborough but currently spends one day a week based out at Kawartha Centre – Redefining Health Aging. It’s because of a pilot project funded by the Centre for Aging and Brain Health Innovation. Kawartha Centre is using pilot project funding to have an embedded Alzheimer Society First Link Care Navigator (Lisa Hughes) as part of the clinical visit for persons diagnosed with dementia.
Lisa Hughes joins us to talk about the pilot project thus far, what it means to the Alzheimer Society and especially what it could mean for newly diagnosed patients and caregivers.
