Israel blames aid worker deaths on ‘operational failures’

Israel is blaming serious errors and breaches of procedures for the attack that killed seven humanitarian workers, including a dual Canadian-American citizen, in Gaza on Monday. The Israeli military has disciplined at least three officers for their roles in the strikes. But as Jackson Proskow reports, accusations are growing that Israel isn’t doing enough to protect civilians.
