CENTENNIAL, Colo. — Prospective jurors in the Colorado theatre shooting case were asked in a 75-question survey about whether they had experienced mental illness and their views on the death penalty.

Judge Carlos A. Samour Jr. released the questionnaire Wednesday at the request of media organizations, including The Associated Press, the day after 12 jurors and 12 alternates were seated for the death penalty trial of James Holmes.
Holmes has pleaded not guilty by reason of insanity to killing 12 people and injuring 70 others in a July 20, 2012, attack on a Denver-area movie theatre. If jurors convict him, they must also decide whether he should be executed.
The questionnaire asked about candidates’ opinions on the relationship between mental health and the criminal justice system. It also asked about their views of Holmes.
